Last time I talked about learning Traditional Chinese Medicine at UTS as a first year student and how it was taught using Pinyin. Today I thought instead of talking about 2nd year, this time I would focus on the University clinic and what it was like as an intern student.

As I mentioned before our course provided us with a very unique and invaluable learning tool in the form of the student clinic. This clinic was located inside the university at Broadway just opposite the ABC building on Harris Street. It was conveniently located in the middle of all our other classes so it was very easy to get there after class and for those that are unfamiliar with UTS, the university it located in the City with campus buildings spread between Broadway and China Town and our university library is next to Market City.

Why was the clinic special? Well for starters most university internships are generally towards the later years of the course and you would generally intern at an external facility like at an office or hospital etc. Instead of that we had our own clinic called the UTS Student Clinic. It operated like a proper clinic and business and was opened to the public. We took walk-ins as well as bookings and it is where we developed most of our practical knowledge that we learnt in the classrooms but most importantly it was a clinic that was almost purely run by the students from the first year through to the fourth year.

I have mentioned a few times previously that we start the internship practically within the first few weeks of the course and I keep mentioning this because of how impactful the experience the clinic had on us and let us get a real taste of what it is like to operate a Chinese medicine clinic as a practitioner as well as administrative staff.

So how did the clinic operate? Well, the clinic was opened throughout the year during the weekdays and would only close during public holidays and between the 2 semesters. As the clinic is run with the combined effort and manpower of teachers and 4 years total of students this is not an issue. The clinic days would be divided into morning and afternoon shifts and you would be allocated to one of them on a roster. The clinic opened up at 7:30am and runs till 1pm and the afternoon shift is from 12:30pm to 6pm. I don’t really remember how our rosters were set in the first 3 years but in the 4th year we had 2 shifts each week until we met our intern quotas.

For the first 2 years we mostly just shadowed the attending practitioner who was on clinic duty that day and observed the consultation and acupuncture treatment. We would be in groups of 5-6 following 1 teacher while at the same time there would also be 2nd,3rd and 4th year students working as well. The most we did was taking turns feeling the pulse and writing notes during the consultation. During acupuncture sessions it was strictly observation only in first year.

The most fun part was when we had to prepare the herbal formula. So obviously we would be under constant supervision especially if it meant the students got physically involved but preparing the prescription is fairly simple. We enter the teacher’s formula into a computer which then generated a label with the prescription and patient details and after we packaged the herbs up we would stick the label on like similar to a pharmacy. Due to the large number of students and with other classes to attend, we only had to do 5 shifts per semester in the first year, the minimum quote was 25hrs which does not sound like a lot but we also spent more time in class then and by the final year we had at least 150hrs that year alone. In total we had almost 300hrs of internship where on average other courses would be between 100-180hrs.

From the 2nd year onwards we would be developing and building on the previous year’s knowledge and we would also start to take on more duties. We had to do the consultation ourselves and then provide our notes to the teacher who would of course follow up with the patient to confirm our diagnosis. From the 3rd year the Tuina clinic is solely run by 3rd year students so in addition to our acupuncture and herbal shifts we would also be the ones doing all the massage using all techniques we learn from one of our teachers called Dale who was a Westerner with a very strong interest in doing research on ancient texts.

The 4th year is when it really starts getting more serious as the operation of the clinic was pretty much handed over to us and I’m not just talking about seeing our own patients or carrying out the treatments but the administration and business side of maintaining the clinic as well. So prior to this each student is also rostered to work as the receptionist for the clinic throughout the course and we were essentially the face of the clinic to the public. We took bookings and finalised payments and at the end of the day we would balance the register and close up. The students rostered for the day are also responsible for cleaning the clinic and getting it ready for the next shift.

In the fourth year we were responsible for the market and advertising (using the university’s funding) the clinic as well as come up with new plans and strategies with making sure the clinic continues to develop and grow into a better teaching tool. The class was split into different groups that oversaw different aspects of the clinic. There was the rostering team that was responsible for figuring out the shifts for everyone, marketing team and advertising teach that worked closely with each other to hold events and produce advertisements, daily management maintained the clinic as supervisors as well as improve performance and efficiencies in the clinic and then there was the disciplinary team that created the rules and handed out infractions.

I was on the daily management team and helped put together a set of guidelines and list of duties everyone must perform as well as created the role of a shift supervisor who would help keep everyone in line. We also created guidelines on how to allocate treatment beds and booking practices as in the previous years they did not have a good enough system and made bookings a bit messy.

One of the most memorable changes me and my team made involved a prior incident with a student and blood letting. This particular student was very fond of blood letting as a treatment and one time during 3rd year they made such a bloody mess so we had to specifically introduce new rules for them. Usually during blood letting we would, after sterilising the area, lance the acupuncture point and express out some blood which was very good for reducing heat instantly and sometimes we would combine it with some cupping to draw out more blood especially on the back. This mean that there would be some blood contaminated glass cups which we had to dispose of into a biohazard container for cleaning and sterilising via 3rd party company. This particular incident traumatised some of the students working that day as they found 15-20 glass cups completely filled with thick jelly like blood and was inappropriately discarded on the treatment equipment trolley and the general waste bins in the storeroom. Obviously, the student responsible was reprimanded and as such my team came up with more stringent safety and disposal protocols to prevent this from happening again.

Overall the entire 4 years intern experience was the of the best times I had at uni as it not only provided important practical experience it also showed me firsthand what it would be like to run my own clinic. We saw many patients during the 4 years and got to know some of our patients very well and I believe it is an experience that you will be hard pressed to find in another course.
